First Session Forms
Please complete the following before the first meeting. This saves time in the first session so we can have the most effective start in addressing your concerns. Sessions are 45 minutes in length.
1. Client Information Questionnaire All persons age 12 and older attending must complete forms ONLY ON THEMSELVES. If you are coming for your child, do not complete one of these for your child.
2. Written Goals This is very important, to be done by each person coming over the age of 10.
3. Marital Requirements (for marriage counseling only)
3. Insurance Verification (Required) This requires a phone call to your insurance company. The only way to have accurate information to avoid denial of coverage is to make this call! I will work with your insurance company, but do not have the time to call every plan for this information myself. Your help is greatly appreciated.
