Please complete the following before the first meeting.
This saves time in the first session so we can have the most effective start in addressing your concerns. Sessions are 45 minutes in length.
PRINT AND COMPLETE ALL FORMS BELOW
- Client Information Questionnaire (one per person required) All persons attending should complete. If your child is too young to do this, parents should complete this form ABOUT THEMSELVES, NOT THE CHILD.
- Client Rights (one form required per household)
- Written Goals This is very important, to be done by each person coming to the counseling session.
- Insurance Verification (Required if applicable) This requires a phone call to your insurance company. The only way to have accurate information to avoid denial of coverage is to make this call!
